1. Scenario
   As a manager on a medical surgical unit, part of the job responsibility includes chart audits.
   A trend noted during the most recent audit was the overuse of the term “dementia.” The nurse manager notes that there may be a lack of understanding between the types dementia as related to neurological diseases.
   The nurse manager developed a survey to evaluate the staff’s understanding of the different types of dementia. As a result of the survey, you have been asked to create a PowerPoint presentation to increase the nurse’s awareness of the types of dementia a presentation was created.
   Instructions
   Choose one of the following topics for a focused presentation:
   • Alzheimer’s
   • Parkinson’s
   • Vascular Dementia
   • The presentation should include the following:
   • Pathophysiology
   • Etiology & incidence
   • Health promotion and maintenance
   • History/Risk factors
   • Physical signs and symptoms
   • Changes in cognition
   • Changes in behavior & personality
   • Changes in self-management skills
   • Diagnostics: laboratory and imaging assessment
   • Planning and implementation
   • Interprofessional Collaborative Care
   • Psychosocial integrity
   • Medications
   • Safety considerations
   • Using Ignatavicius and one additional resource, develop a presentation to enhance the nurses’ knowledge of the differences between types of dementia and delirium. Be sure to document your source(s) in your presentation.

Students are required to submit weekly reflective narratives throughout the course that will culminate in a final, course-long reflective journal due in Topic 10. The narratives help students integrate leadership and inquiry into current practice.

This reflection journal also allows students to outline what they have discovered about their professional practice, personal strengths and weaknesses, and additional resources that could be introduced in a given situation to influence optimal outcomes. Each week students should also explain how they met a course competency or course objective(s).

In each week’s entry, students should reflect on the personal knowledge and skills gained throughout the course. Journal entries should address one or more of the areas stated below.  In the Topic 10 graded submission, each of the areas below should be addressed as part of the summary submission.

Topic: Theory – Practice Gap

Think back to the beginning of the course and consider your thoughts on learning about nursing theory. Did you question the usefulness of spending a term learning about theory? Next, consider your current practice environment now that you know more about theory. Is theory used in practice, or is there a theory practice gap that needs to be addressed?

Begin by identifying a patient from your clinicals who has a chronic health condition such as diabetes, COPD, hypertension, mental health diagnosis, and post stroke care. Discuss all aspects of healthcare coordination with the chosen diagnosis. Your paper should include the following information:

1. Description of the patient’s health issues, diagnoses, and treatment plan.

2. Description of the interdisciplinary team that will participate in this care.

3. Can the CCCR model be used in managing this patient’s care? How so? If not, can you name another model to help in managing this patient’s care?

4. What is the cost of managing this disease? Are there resources available for the patient and family to help with costs and other factors such as commute and etc?

5. What is your role as the APRN in this team?

6. As the APRN, how can you evaluate if the care management is successful?

7. As the APRN, how can you evaluate if this patient needs a higher level of care?

8. Are there any barriers or challenges that you foresee for this patient? How can you overcome these barriers?

· The body of your paper should be a three to four pages long. This does not include the title page and reference page. Papers that are more than four pages long will not be accepted. Incorporate information from the course text plus a minimum of three (3) scholarly references published within the last five (5) years. Textbooks are not to be considered scholarly references.

· Use current APA Style to write your paper and to cite your sources. Your source(s) should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas.

Case Study: Jimmy, ten years old, was admitted to the pediatric intensive care unit after a fall from the second-story townhome were sustained a fractured left femur and mild head injury. Currently, Jimmy is two days post open reduction internal fixation of the left femur. Orders were updated to transfer Jimmy out of Intensive Care Unit (ICU) after being cleared by the neurologist. He has a long leg cast, indwelling foley catheter and will require neuro checks every two hours.
