Scenario

You are a registered nurse (RN) working in a Women’s OB/GYN Clinic. Elizabeth Jones, 37 years old, presents to the prenatal clinic after missing her last 2 menstrual cycles. Her home pregnancy test was positive. An ultrasound at the clinic confirms pregnancy. Gestational age is calculated to be 10 weeks. An initial assessment of Ms. Jones’s medical and obstetrical history is as follows.

Obstetric/Gynecologic (OB/GYN) history: Uncomplicated spontaneous vaginal delivery at 39.2 weeks (3 years ago); Cesarean section x 1 at 37.5 weeks for non-reassuring fetal heart tones (1.5 years ago); abnormal Papanicolau (PAP) smear x2, + human papilloma virus (HPV), colposcopy within normal limits

Medical history: Chronic hypertension (HTN) x 5 years;

Allergies: Penicillin

Social history:

· (+) tobacco, “occasional” per client (pt), <5 per/day currently, has smoked “off and on” for 15 years

· (+) cocaine use, states she has not used any cocaine/drugs for > 1 year; (-) alcohol use

· Abusive partner with first pregnancy, states she has a new partner x 4 years

· Depression, currently not taking meds for treatment (tx)

Medications: Prenatal vitamins; Labetalol 200mg BID;

Family history: Insulin-dependent diabetes mellitus (mother); HTN and heart disease (father); breast cancer (maternal grandmother, deceased)

Write a three-page analysis of this scenario that answers the following questions:

1. What should the nurse consider related to caring for a client with a history of domestic abuse, drug use, sexually transmitted diseases and depression?

2. Document the considerations of yourself as the professional nurse in regards to self-awareness; be aware of attitudes, values and beliefs that you hold related to clients from different social backgrounds so that care is not affected negatively.

3. What conditions are in Mrs. Jones history that would cause concern during pregnancy, labor, and birth?

4. What concerns should be discussed with Ms. Jones before she leaves her appointment?

Hello Class,

In my future nursing practice, I have identified “Evidence-Based Practice” and “Teamwork and Collaboration” as the two Massachusetts Nurse of the Future Nursing Core Competencies that will have a significant impact. By utilizing evidence-based practice, I can ensure that my nursing interventions are backed by current studies and clinical data. This approach allows me to critically evaluate data and incorporate it into my decision-making process, ultimately improving the standard of care I provide. Additionally, evidence-based practice promotes ongoing learning and development, enabling me to stay up-to-date with the latest developments and make decisions that best serve my patients' well-being. Teamwork and Collaboration are equally crucial, as they reflect the interconnected nature of healthcare delivery. Effective teamwork ensures seamless patient-centered care, which is essential in nursing practice, given the need to work alongside various healthcare professionals. Collaborating with diverse team members enhances my ability to gather varied perspectives, share knowledge, and collectively address complex patient needs. This competency fosters open communication, mutual respect, and a shared commitment to achieving optimal patient outcomes (Masters, 2021).

The intersections between both competencies are evident, as evidence-based practice often requires collaboration among interdisciplinary teams to implement new findings and collectively improve patient care. By effectively utilizing evidence and collaborating with interdisciplinary teams, I can ensure that my nursing practice is not only evidence-driven but also inclusive and aligned with broader healthcare goals.

The AACN BSN Essential of Person-Centered Care is the most crucial feature that will significantly improve my future nursing profession. These essentials align with the core values of nursing and embody the holistic and empathetic attitude necessary for providing excellent patient care. Person-centered care promotes personalized, comprehensive care that takes into account each patient's unique requirements, preferences, and experiences. By focusing on the person's family, societal, and cultural circumstances, nurses can deliver just, respectful, and evidence-based care. Patient-centered care is the foundation of nursing practice because it strengthens nurse-patient relationships, encourages shared decision-making, and enables patients to participate in their treatment (AACN, 2021).

In my practice, person-centered care will improve patient experience and health outcomes. I can personalize my care plans to my patients' requirements by learning and respecting their diverse backgrounds, attitudes, and beliefs. This strategy targets physical, emotional, psychological, and social well-being. I also believe in healthcare team communication, empathy, and teamwork, which person-centered care supports (Masters, 2021b).

Finally, the AACN BSN Essential of Person-Centered Care aligns with my nursing ideals of providing holistic, compassionate, and tailored care that recognizes each patient's uniqueness. By embracing this fundamental strategy, I can improve patient outcomes and humanize healthcare by offering respectful, empowering, and values-aligned care.

Barrier #2; Nurses are not visible in healthcare policy making. 

According to Anders (2021), nurses are professionals who are directly involved in patient care. Engagement front line nurses have with patients, and other healthcare professionals, make them ideal healthcare policy advocates. They have appropriate academic education and clinical expertise to serve as experts in sharing their knowledge with policymakers and others. Any other profession cannot replicate the situational knowledge of the healthcare needs of patients that this large workforce offers.  

However, nurses’ access to policy-making processes is limited. The author supports the concept of barrier by explaining that despite nursing’s intrinsic involvement in the advocacy of the individual patient, the nursing voice is often not heard at a public level. Since nurses represent approximately 50% of the global health workforce, they should be actively involved in policy-making processes. However, nursing representation at the government and boardroom level is still slim (Anders, 2021).  

The author suggests how nurses can participate in policy development. Meaningful policy change can occur at the institutional level, where nurses are accustomed to functioning. Nurses’ involvement in self‐governance within hospitals presents opportunities for staff nurses to participate in the clinical governance at a local level. Nurses can also begin as a member contributing to online discussions, blogs, or social media interest groups sponsored by the association, then progress to active participation in any number of subcommittees, and finally run for office.
