Hello Everyone,

In a health facility, it is important to ensure that patients morals and values are being obtained. Many people come from different cultural backgrounds and it is crucial to be able to understand them and respect them the best way possible. You want to make sure everyone feels safe and comfortable between the patient and the nurse. Its important to make sure the patient feels respected with whatever healthcare decisions they decide to make.

1.What values ensure a culture of safety?

Like in most situations, communication amoungs co-worker and patients is key to gather the proper information about the patient. It is crucial that we have a strong and well established team that makes working as a team a priority for every patient. We have to value everyone decisions and what they think is best for themselves. Culture plays a huge role in the patients decision making for themselves. It is important that as a team we work together to sustain a safe environment and to make sure everything is organized. Having great organization skills keeps the environment stay at a better state and avoids any problematic risk factors that can potentially harm the patient in any way.

2.How can healthcare facilities establish a culture of safety?

Some ways is establishing great teamwork and great work ethic practices. As nurses, we know to use EBP to ensure that the patient is in the best care needs from us. Working as a team will help maintain the environment the more nurses that work together the better the outcomes. Everyone in the facility should be promoting safety to make the environment welcoming.

3.What is the nurse's role in maintaining a culture of safety?

We all know that every hospital is going to have different set of rules depending on the state and region. Although, most hospitals will have very similar rules that nurses and medical professionals must follow. Many of these hospitals have to follow government advised rules like using EBP for every patient. I still think every single nurse should be educated in different cultures to understand every patient and for them to be well respected. Nurses play a huge role in taking proper care of each unique individual. Actively being part of ANA, taking every protocol seriously and maintaining a safe work environment for other staff and for the patient. As nurses, we play a vital role in creating a culture of safety within our clinical work area by maintaining an open line of communication with our healthcare team members, assessing our peers' and our own professional work habits and behaviors for safety risks, utilizing evidence-based practice (EBP) interventions in our patient protocols, and participating in shared governance workgroups (Davis,2017).

In a health facility, it is essential to produce the culture of safety. It is important for workers within the healthcare facility to have respect for one another, especially for everyone to feel safe with each other. Not only that, but it's crucial for a facility's culture to support and promote patient safety.

1. What values ensure a culture of safety?

After reading, I have learned that there are various values that ensure a culture of safety. One example is the value of teamwork. The team needs to work together in order to provide each patient with a consistent safe experience. Another value is respect. As I mentioned earlier it is important for healthcare workers to respect one another but it is just as important for healthcare workers to be nothing but respectful to their patients. Another value includes organization and planning. It is important to acknowledge what can cause the culture of safety and plan accordingly.

2. How can healthcare facilities establish a culture of safety?

Some examples include teamwork by healthcare workers. It is important for leaders to be visibly committed to change. It is also essential for the full staff to openly share safety information with one another. Everyone should be on the same page of promoting patient safety.

3. What is the nurse's role in maintaining a culture of safety?

I understand that every health facility is ran differently and has different standards. I believe though, that every nurse should maintain a culture of safety. I feel that they have a crucial role because they are the main caregivers of the patients when they come to the facility. The patient is allowing the nurse to take care of them by putting a lot of trust in them. Due to this trust, nurses need to make it a goal to keep them safe at all times. The nurses are in control as far as safety because they are aware of the facility and regulations. If the nurse does a successful job at maintaining a culture of safety, it will create a safe and trusting experience between the nurse and the patient.

The suggested disease in the case study is protein malnutrition. It is a condition where the patient does not have adequate protein intake. Genetic disorders can cause protein malnutrition. For instance, cystic fibrosis can lead to pancreatic insufficiency, limiting the production of digestive enzymes necessary for protein digestion (Montoro-Huguet et al., 2021). Celiac disease can lead to malabsorption of many nutrients. Hartnup can hinder the absorption of Tryptophan, which is necessary for protein synthesis (Schmidt-Kastner & Kravetz, 2022). However, these genetic factors may not be adequate to cause protein malnutrition. Other health conditions and poor dietary intake may also play an important role.

The patient presents with generalized edema of extremities and abdomen due to low levels of albumin (a plasma protein), which causes the leakage of fluids from the blood vessels into the surrounding tissues due to low osmotic pressure. The physiologic response to the stimulus is an attempt for the body to maintain homeostasis amid the lack of adequate protein. The body breaks down muscle tissues to release amino acids necessary for several cellular processes. The process causes muscle wasting and exacerbates protein deficiency.

Liver cells (hepatocytes) are involved in protein malnutrition. Hepatocytes synthesize plasma proteins, including albumin. Due to inadequate protein, these cells do not produce adequate albumin. Therefore, oncotic pressure in the blood vessels decreases, leading to edema (Grüngreiff et al., 2021). Another characteristic that would change the response is gender influence. The female elderly patient may have low levels of estrogen that would cause hormonal imbalance. Therefore, there would be a decrease in muscle mass and protein synthesis, a possible cause of protein malnutrition. The patient’s progesterone levels may also fluctuate due to menopause, leading to poor nutrient intake and loss of appetite. These gender-based factors may cause protein malnutrition.

Technology is an ever-changing and advancing phenomenon that can be tricky to navigate. “Society has moved from a world dominated by paper and face-to-face interactions, to an online presence and way of communicating that is in a constant state of evolution” (Knox, 2019, p. 1). Whether the nurse be young or older, I believe that navigating technology can come with growing pains for everyone. Technology is constantly updating and progressing into programs that aid in essential parts of nursing informatics. “Informatics knowledge and skills provide the tools to understand and use technology to work more efficiently and effectively, and to work smarter, not harder” (Chamberlain College of Nursing, 2020). The nurse must be able to use technology effectively in order to provide safe and efficient patient care. Clicking around the EHR can be overwhelming to some, as there are a lot of different ends and outs of different health care programs. It is essential for the nurse to not only be able to chart on their patient, but to be able to obtain proper data and information on their patients in order to perform interventions appropriately. Nursing informatics involves the DIKW theory– which is collecting and applying data, information, knowledge, and wisdom through technology. Through nursing informatics, I believe I am going to be learning a lot about how to connect the dots through patient care in a way that benefits communication in nursing and patient safety using technology. For seasoned nurses this may be less intimidating, but I feel that there is always going to be room to grow and learn when it comes to informatics no matter the skill set. Although I feel confident in using technology I scored myself a 32 overall. I believe that I could use further education on a few categories as I scored myself “good”, but was reassured to see myself as “very good” in others. I am excited to be improving on things like recognizing the role of information technology in improving patient care outcomes and creating a safe care environment throughout this course. I don't believe that I have any additional questions at this time as I feel that the lessons and readings have provided me with great context of what this course has to offer so far. I will utilize my teacher, and the Q&A forum if I do end up having any questions.
